About the role
Key responsibilities & impact- Serve as a primary point of contact for patients, family members, and referring physicians to access care
- Decrease fragmentation of care and coordinate services
- Serve as a clinical liaison for new and existing patients
- Assist with administrative functions and development and implementation of patient care protocols, clinical pathways, and patient policies and procedures
- Coordinate treatment efforts and promote quality improvement and educational efforts
- Help patients and family members navigate medical care and find solutions to treatment-related problems
- Assess and monitor continuity of care and expedite and coordinate service delivery
- Collaborate with the medical team for follow-up and monitor patient progress
- Participate in development, implementation, and review of quality metrics
- Develop and implement patient education materials
- Identify indicators to monitor and track patients, referral outcomes, and campaigns
- Compile data, track outcomes, and recommend process improvements
- Identify and report quality issues; engage in quality assessment, utilization review, risk management, and infection control
- Provide referrals and follow-up support for uninsured or underinsured patients
Requirements
What you’ll need- Specialized Diploma Nursing
- 2 years experience working as an RN
- Ability to maintain confidentiality of records and information
- Knowledge of patient education techniques and principles
- Knowledge of community resources and how to access them effectively and efficiently
- Skills and knowledge in obesity management theories, practices, and techniques
- Proficient in Microsoft Office applications
- Skilled in both medical and psycho-social assessment
- RN – Licensed Registered Nurse, Pennsylvania state license, upon hire
- Lift and carry 25 lbs.; frequent sitting/standing and keyboard use; patient care providers may need to kneel, bend, squat, and perform CPR
- Bachelor's Degree in Nursing preferred
