About the role
Key responsibilities & impact- Navigate and manage complex chronic comorbidities
- Create and implement treatment plans for patients and family caregivers

- Provide high-quality customer service
- Provide clinical and medical education to clients and staff
- Participate in virtual care conferencing
- Update and maintain medical records
- Participate in quality improvement initiatives and develop best practices
- Contribute to the strategic direction of Nolia Care services
- Provide virtual telehealth care to senior patients with chronic conditions and their family caregivers
Requirements
What you’ll need- Two or more years outpatient experience in independent private practice or within a larger medical center
- Experience utilizing chronic, longitudinal care models in digital health required
- Experience in FNPs (Family Practice) and AGPCNP (Adult Geriatric Primary Care) preferred
- Nurse Practitioner and Registered Nurse licenses in CA state required
- WA, TX, ID, FL, AZ and/or multistate licensure also needed
- Board certification as a Nurse Practitioner in geriatric care preferred
- DEA required
- National board certification as a Nurse Practitioner from an accredited agency
- Master’s level ARNP required
- Additional training or experience in Geriatric care, Dementia and Palliative specialties
- Doctorate of Nursing preferred
- Strong interpersonal, leadership and problem-solving skills
- Strong commitment to providing excellent, team-oriented care
