About the role
Key responsibilities & impact- Provide ongoing support and expertise through comprehensive assessment, planning, implementation, and evaluation of individual patient needs
- Coordinate care and discharge planning for all patients
- Integrate social services and case management functions into patient care, discharge, and home planning processes
- Coordinate with external service organizations, agencies, and healthcare facilities
- Act as patient advocate
- Investigate and report adverse occurrences
- Evaluate patient satisfaction and quality of care
- Review appropriate utilization of services from admission through discharge
- Ensure patient tests are appropriate, necessary, completed on time, and results are promptly available
- Provide staff education on resource utilization, discharge planning, and psychosocial aspects of healthcare delivery
- Communicate regularly with physicians and develop effective working relationships
- Assist physicians in maintaining appropriate cost, case, and desired patient outcomes
- Participate in developing and implementing patient care policies and protocols
- Perform other assigned duties
Requirements
What you’ll need- Three (3) years of nursing experience in a clinic or hospital setting, preferably including one (1) year in a related patient population such as utilization management, chronic disease management, home health, or hospice
- Associate's Degree degree in nursing is required
- Knowledge in a supervisory or quality assurance position in a healthcare environment
- Case management experience
- Knowledge as a Registered Nurse in a clinical care setting
- Good interpersonal skills
- Influencing and motivational interviewing skills
- Ability to speak, read, write, and communicate effectively
- Ability to coordinate, analyze, observe, make decisions, meet deadlines, and work in a detail-oriented manner
- Ability to work independently without supervision
- Ability to manage multiple priorities with effective organizational and time management skills
- Strong teamwork skills
- Effective computer skills using Microsoft Office (Word, Excel, PowerPoint, etc.) and database software
- Demonstrated aptitude and/or proficiency with Electronic Medical Record and Case Management software
- Epic experience highly desirable
