About the role

Key responsibilities & impact
  • Establish and maintain connections with community partners, healthcare providers, and relevant organizations
  • Promote the hospital's services and build relationships
  • Facilitate seamless patient transitions between community and hospital settings
  • Develop and implement outreach strategies for community organizations, physician practices, clinics, senior living facilities, and healthcare providers
  • Build referral pathways and promote specialized hospital services
  • Educate community partners and healthcare providers about services, programs, and patient resources
  • Collaborate with physicians, discharge planners, case managers, and other healthcare professionals to facilitate referrals
  • Conduct assessments to identify healthcare needs and gaps in the community
  • Organize and participate in community events, health fairs, seminars, and workshops
  • Collaborate with marketing and public relations teams on targeted outreach materials and messages

Requirements

What you’ll need
  • Liaison experience preferred
  • IRF/acute rehab experience preferred
  • Active clinical license in Massachusetts required (RN, PT, OT, SLP, LCSW/LICSW)
  • Bachelor's Degree in Healthcare Management, Public Relations, or Related Field of Study required
  • 2–3 years of community outreach experience required
  • 1–2 years of healthcare/managed care experience required
  • Knowledge of local healthcare resources and community organizations
  • Familiarity with healthcare regulations, privacy laws, and ethical standards
  • Strong communication, presentation, and interpersonal skills
  • Ability to establish and maintain positive relationships with varied stakeholders
  • Excellent organizational and time management abilities
  • Valid driver's license and willingness to travel within the community as needed
