About the role
Key responsibilities & impact- Collaborate with healthcare professionals to evaluate patients' medical or physical conditions and assess client needs
- Provide psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues
- Formulate biopsychosocial assessments, dispositions, and treatment plans
- Develop and implement patient-centered treatment and care plans as part of an interdisciplinary team
- Monitor, evaluate, and record client progress according to measurable goals
- Bring patient needs, safety measures, and concerns forward
- Support onboarding for new team members as needed
- Participate in department and entity-level service initiatives and use data to measure progress
- Attend team meetings twice monthly at the Assembly Row office in Somerville, Massachusetts
Requirements
What you’ll need- Master's Degree Social Work or Master's Degree Mental Health & Behavioral Medicine required
- Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) required
- 0-1+ years of clinical experience in medical setting required
- Pediatric experience strongly preferred
- Bilingual (Spanish/English) strongly preferred
- Excellent organizational and time management skills
- Excellent crisis intervention skills
- Good problem solving and conflict resolution skills
- Ability to work well collaboratively and independently
- Strong written and verbal communication skills
- Knowledge of community resources and the aging process
- Fundamental skills in formulation, risk assessment, crisis intervention, psychosocial treatment plans, and documentation
- Developing interpersonal collaboration in an inpatient or outpatient medical setting
- Monday through Friday ET availability, 8am-4:30pm or 8:30am-5pm
- Flexibility to support on-site needs in Chelsea, Revere, and Charlestown approximately 2-3 times per week, with possible patient-home or school meetings
- Stable, secure, quiet, compliant workstation for remote work
