About the role
Key responsibilities & impact- Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients
- Provide psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues
- Formulate biopsychosocial assessments, disposition plans, and treatment plans
- Collaborate with healthcare professionals as part of the interdisciplinary team
- Evaluate patients' medical or physical conditions and assess client needs
- Monitor, evaluate, and record client progress against measurable treatment and care-plan goals
- Develop and implement patient-centered treatment plans
- Provide evidence-based interventions
- Advocate for clients while maintaining professional boundaries
- Refer clients appropriately using internal and external resources
- Engage in quality improvement projects and use data to measure progress
- Facilitate skill-based groups
- Use supervision and consultation when appropriate
Requirements
What you’ll need- Master's Degree Social Work (MSW) required
- LCSW, LICSW or LMHC licensure required
- 2+ years clinical experience in a medical setting required
- Proficient clinical judgment and application of professional values and ethics
- Proficiency in risk assessment, crisis intervention, psychosocial assessments, treatment plans, and timely documentation
- Knowledge of internal and external resources
- Ability to advocate, coordinate, and communicate treatment plans with patients, families, and interdisciplinary teams
- Ability to facilitate skill-based groups
- Knowledge and ability to work in a complex organization
- Capacity to effectively communicate findings with a broader audience
- Stable, secure, and compliant workstation in a quiet environment for remote workdays
