About the role
Key responsibilities & impact- Conduct in-depth assessments of patient needs, goals, and barriers to achieving good health outcomes
- Provide advanced health education and coaching to individuals and families
- Collaborate with healthcare providers to develop and implement care plans for patients with complex health needs
- Assist patients in navigating the healthcare system and accessing health insurance and social services
- Provide ongoing follow-up and support to ensure continuity of care and achievement of health goals
- Develop and implement health promotion programs and activities addressing community health needs
- Collect data and maintain accurate records of patient interactions and outcomes
- Attend meetings and trainings related to community health promotion and education
Requirements
What you’ll need- Bachelor's Degree in Public Health required, or Bachelor's Degree in Social Work required, or Bachelor's Degree in a related field of study required
- Experience in community health outreach, health education, or related field: 2–3 years required
- Community Health Worker [CHW - Massachusetts License] preferred
- Ability to work effectively and advocate for all populations and communities
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Basic computer skills, including Microsoft Office and database management
- Bilingual skills preferred
- Ability to perform the listed physical requirements
