About the role
Key responsibilities & impact- Conduct community outreach and engage community members to understand their health needs
- Provide individual and group health education and coaching
- Assist community members in accessing and navigating the healthcare system
- Provide referrals to appropriate health services
- Provide basic health screenings and assessments to identify health risks and appropriate interventions
- Assist in developing health promotion programs and activities
- Collect data and maintain accurate records of outreach activities and interventions
- Attend meetings and trainings related to community health promotion and education
- Work under the direction of a healthcare professional to provide preventive care and health education
- Support on-site needs at medical practices, patient homes, and schools for meetings
Requirements
What you’ll need- High School Diploma required
- Associate’s Degree or Bachelor’s degree preferred
- Experience in community outreach, health education, or related field strongly preferred
- Bilingual (English/Spanish) strongly preferred
- Pediatrics experience strongly preferred
- Strong communication and interpersonal skills
- Ability to interact effectively with various populations
- Ability to work effectively and provide advocacy for all populations and communities preferred
- Ability to work independently and as part of a team
- Basic computer skills, including Microsoft Office and database management
- Flexible availability to support on-site needs at medical practices in Chelsea and Everett approximately 2–3 times per week, with variation based on case load
- Attendance at twice-monthly team meetings at the Assembly Row office in Somerville, MA required
- Stable, secure, quiet, compliant workstation for remote work
