About the role
Key responsibilities & impact- Coordinate and support healthcare within the facility
- Coordinate referrals for services outside the clinic
- Provide outreach and enrollment services to meet program and county eligibility requirements
- Coordinate and plan community and family support
- Develop and coordinate community service plans
- Partner with service providers, families, and patients to create healthcare plans
- Conduct client assessments and planning
- Modify patient treatment plans based on patient responses and conditions
- Prepare patients and families for discharge
- Communicate with third-party payers to obtain service reimbursement authorization
- Define patient care goals through education, information, and direction
- Review challenging cases with medical directors as needed
- Maintain accurate, detailed reports and records
Requirements
What you’ll need- Associate's Degree Nursing required
- Active and unrestricted Registered Nurse (RN) Licensure for the state of Massachusetts required
- 2+ years of case management and/or discharge planning required
- Experience working with complex dually eligible populations required
- Knowledge of Managed Care preferred
- Understanding of diagnostic criteria for dual conditions and ability to conceptualize modalities and placement criteria within the continuum of care preferred
- Ability to establish strong rapport and relationships with patients and staff
- Proficient in Microsoft Office and industry-related software programs
- Ability to maintain client and staff confidentiality
- Ability to identify complex problems, evaluate options, and implement solutions
- Ability to use logic and reasoning to assess alternative solutions
- Ability to listen attentively and ask appropriate questions
