About the role

Key responsibilities & impact
  • Maintain a professional image and exhibit excellent customer relations
  • Coordinate access to health care services and provider referrals for low-income uninsured or underinsured clients
  • Assess social drivers of health and facilitate access to care by reducing or resolving barriers
  • Provide care coordination, community outreach, education, navigation, and support services
  • Establish medical homes for primary care and help prevent unnecessary emergency room visits
  • Arrange medical appointments and connect patients with services, programs, and organizations
  • Address medical, psychosocial, and behavioral needs
  • Establish trusting relationships through home, physician office, community, and virtual visits
  • Help patients and families access community services and healthcare resources
  • Provide culturally sensitive health information and promote healthy behaviors and disease prevention
  • Offer social support and informal counseling, helping individuals set and achieve health goals
  • Conduct home visits and attend community meetings and events
  • Act as a patient advocate and liaise with healthcare providers
  • Maintain communication through phone calls and home visits to ensure follow-up care
  • Record patient interactions and care management information in EMR within specified timeframes
  • Provide emotional support, encouragement, and motivation to patients

Requirements

What you’ll need
  • High School Diploma or GED
  • Willing to obtain BLS
  • Community Health Worker Core Competency Certification within first year of hire
  • Strong knowledge of community resources and services
  • Ability to work with diverse populations
  • Excellent communication and interpersonal skills
  • Strong organizational and problem-solving skills
  • Ability to work independently and as part of a team
  • Basic computer skills, including familiarity with Microsoft Office and Excel
  • Valid driver's license and reliable transportation
  • Comfortable working in various community settings
  • Demonstrated commitment to improving community health
  • Cultural competency and sensitivity
  • Knowledge of medical terminology required
  • Effective professional verbal communication and documentation skills
  • Must use own car for travel to remote sites, often in rural, isolated areas, with proof of current auto insurance
