About the role
Key responsibilities & impact- Provide virtual care to patients benefiting from ongoing chronic disease management
- Build patient relationships and monitor vital sign trends
- Conduct virtual patient visits between regular office appointments
- Support chronic disease management, discharge follow-up, and acute care needs
- Advance guideline-directed medical therapy (GDMT)
- Facilitate advance care planning for Medicare and MA patients
- Collaborate with primary care clinicians in ambulatory settings to coordinate care
- Perform virtual assessments, patient education, triage, and escalation of care needs
- Assess medically complex patients and acute exacerbations of chronic conditions
- Coordinate care with physicians, advanced practitioners, specialists, social workers, physical therapists, and palliative care providers
- Document care and maintain patient confidentiality
- Practice within the RN scope under the supervision of an advanced practitioner
Requirements
What you’ll need- Active RN license in applicable state(s)
- Experience in chronic disease management and virtual care preferred
- Strong communication and collaboration skills
- Commitment to Mercy’s Nursing Conceptual Framework and patient-centered care
- Must be within 1 hour of Chesterfield Virtual Care Office after training
