About the role

Key responsibilities & impact
  • Coordinate care for assigned inpatient and outpatient populations
  • Guide patients through their treatment journey as a primary resource and advocate
  • Provide patient and family education, including pre-operative preparation and follow-up care
  • Collaborate with physicians, nurses, and multidisciplinary teams
  • Support discharge planning and coordinate post-acute services
  • Monitor patient progress and improve outcomes, patient satisfaction, and continuity of care
  • Assist with length of stay (LOS) management and utilization review activities
  • Participate in quality improvement initiatives and implement best practices
  • Serve as a clinical resource and mentor to care team members
  • Maintain accurate documentation in EPIC
  • Assess care requirements and make recommendations based on severity of illness, intensity of service, and skill level
  • Ensure timely, safe, effective, and high-quality patient care
  • Coordinate patient discharge disposition and follow-up care
  • Hold care providers accountable for professional responsibilities and regulatory adherence

Requirements

What you’ll need
  • Graduate of an accredited School of Nursing
  • Current RN license in the state of practice
  • Minimum of 2-3 years of nursing experience
  • BLS certification
  • Bachelor of Science in Nursing (BSN) preferred
  • ACLS certification preferred
  • Experience in care coordination, case management, utilization management, or patient navigation preferred
  • Proficiency documenting within the electronic medical record (EPIC)
