About the role
Key responsibilities & impact- Conduct patient assessments and evaluations
- Develop and manage individualized treatment plans
- Provide medication management, including MOUD services
- Monitor patient progress and adjust care plans as needed
- Collaborate with multidisciplinary team members to coordinate care
- Educate patients and families on treatment, recovery, and health maintenance
- Maintain accurate and timely clinical documentation
- Provide direct patient care for individuals with substance use disorders
- Work alongside physicians, APPs, and clinical social workers in an integrated care environment
Requirements
What you’ll need- Current Nurse Practitioner or Physician Assistant license
- DEA registration and prescriptive authority as required
- Strong clinical assessment and communication skills
- Preferred experience in Addiction Medicine
- Preferred experience in Behavioral Health
- Preferred experience in Substance Use Disorder treatment
- Preferred experience in Telehealth and/or collaborative care settings
