About the role
Key responsibilities & impact- Support care managers in delivering Tailored Care Management
- Perform outreach, engagement, and follow-up with people supported
- Help individuals make informed choices regarding care, services, and life decisions
- Provide services and supports focused on safety, self-worth, confidence, community connection, self-advocacy, social supports, and communication
- Conduct skill-building activities related to housing, finances, employment readiness, relationships, health risks, and health-condition management
- Assist with Plan of Care/Individual Support Plan goals and comprehensive crisis plans
- Coordinate services and appointments, including reminders and transportation
- Help individuals address barriers to primary healthcare, dental, oral, and vision services
- Link individuals with community-based housing and employment services
- Build person-centered relationships promoting self-determination
- Assist with self-help, advocacy, pre-crisis support, and reasonable-accommodation discussions
- Document encounters and contacts and submit billing documentation
- Attend staff meetings, case conferences, trainings, and other required meetings
- Convey observations and key issues to the supervisor
- Assist with staff orientation, mentoring, and training
- Follow agency policies, procedures, and service-definition guidelines
- Complete other responsibilities assigned by the supervisor
Requirements
What you’ll need- High School Diploma (Required)
- Valid USA driver's license
- Must be 18 years of age or older
- Direct experience providing care for and navigating the Medicaid delivery system on behalf of an individual, with 2 years required; parent/guardian cannot serve as an extender for their family member
- Lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system
- Paid experience performing the types of functions listed in the job description, with at least one year of paid experience working directly with the Tailored Care Management eligible population; 2 years required
- Must meet at least one of the listed lived-experience, parent/guardian experience, or paid-experience pathways
- Ability to maintain required agency, state, and federal training certifications
- Driving and travel may be required
