About the role

Key responsibilities & impact
  • Deliver primary care in clinic and community settings
  • Provide assessments, preventive screenings, chronic disease management, acute care, and urgent interventions
  • Conduct physical exams and review prior treatments
  • Order diagnostic tests and referrals
  • Prescribe medications and develop person-centered care plans
  • Provide health education to individuals and families
  • Collaborate with behavioral health providers, hospitals, outside primary care clinics, public health departments, and community agencies
  • Support transitions from hospital or emergency settings back into the community
  • Participate in care planning for high-risk or complex individuals
  • Maintain timely and accurate records in the Electronic Health Record (EHR)
  • Attend required meetings and trainings
  • Perform additional assigned responsibilities
  • Travel for community-based care delivery

Requirements

What you’ll need
  • Master's of Science in Nursing, Nurse Practitioner (NP), Psychiatric Mental Health Nurse Practitioner (PMHNP), or Physician Assistant Studies (MSPAS) required
  • Board-Certified Physician Assistant (PA) certification from the National Commission on Certification of Physician Assistants (NCCPA)
  • State license as Nurse Practitioner (NP) from the State Board of Nursing
  • State license as Physician Assistant (PA) from the State Medical Board
  • Experience in a primary care practice setting; 1 year listed but not required
  • Must maintain certifications and licensure
  • Must adhere to agency policies, procedures, and training requirements
  • Must be available for a 40-hour work week
  • Occasional travel for home/community visits required
