About the role
Key responsibilities & impact- Act as a liaison between patients, professional staff and physicians
- Provide care management and coordination for adult patients with complex conditions, social needs and educational needs
- Collaborate with primary care providers, specialists, patients and families
- Assess, plan, implement, coordinate, monitor and evaluate options and services
- Optimize patients' health status and decrease hospital and emergency room utilization
- Support quality improvement initiatives and adherence to safety standards
Requirements
What you’ll need- Licensed RN in the State of Michigan
- Experience in complex chronic disease management preferred
- Bachelor of Science degree in nursing preferred
- Five years' experience as an RN preferred
- Must be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella
- Must receive a flu vaccine during the flu season in the year hired and annually thereafter, or have an approved medical or religious exemption
