About the role

Key responsibilities & impact
  • Provide nursing care services to residents/clients under the general supervision of the Director of Operations- Advance Practice
  • Conduct physical examinations and comprehensive medical and social histories
  • Make independent decisions regarding management and treatment of medical problems
  • Order, perform, and interpret laboratory tests for screening and diagnosis
  • Prescribe and order diets, medications, physical therapy, and other treatments
  • Perform therapeutic or corrective measures, including urgent care
  • Refer individuals to specialist nurses, physicians, or other healthcare providers
  • Provide health maintenance, health promotion, illness prevention, illness management, and medication counseling
  • Evaluate counseling effectiveness and follow up with noncompliant patients
  • Manage client care, develop treatment and follow-up plans, monitor progress, and arrange referrals or hospital/emergency care when needed
  • Assess social/economic factors and tailor care accordingly
  • Connect clients with appropriate community resources
  • Maintain acceptable productivity to meet facility goals
  • Conduct client home visits and provide in-home care as necessary
  • Accept and handle after-hours calls and problems on a rotating schedule
  • Develop health education materials and clinical practice guidelines
  • Obtain informed consent when appropriate
  • Participate in community programs, health fairs, school programs, and workplace programs
  • Represent the practice or profession as an NP before governing bodies, agencies, and private businesses as needed
  • Collaborate on policy creation and problem solving
  • Perform other duties as assigned

Requirements

What you’ll need
  • Graduate of an accredited school of nursing with a Master’s Degree in Nursing and a nurse practitioner certification
  • Minimum of two years as an RN in a healthcare setting
  • Current RN License and Nurse Practitioner in good standing for the state in which practicing
  • Must have good communication, comprehension, and interpersonal skills
  • Must have the ability to speak, read, write and understand English
  • Must have working knowledge of Windows, Microsoft Office Suite, and World Wide Web
  • Ability to conduct home visits and provide care in the home as necessary
  • Ability to accept after-hours calls and handle after-hours problems on a rotating schedule
  • Moderate travel
  • Must complete annual education requirements, including Compliance, Privacy, Security and HIPAA training
  • Must adhere to organizational policies, the Code of Conduct, facility handbook requirements, and applicable laws and regulations
