About the role

Key responsibilities & impact
  • Provide direct patient care according to the State Nurse Practice Act and physician orders
  • Implement the plan of care initiated by the registered nurse
  • Provide accurate and timely documentation
  • Assess patients and provide patient and family/caregiver education
  • Coordinate home health services and report needs for other disciplines
  • Mentor, coach, and educate direct care staff under RN supervision
  • Use equipment and supplies effectively and efficiently
  • Participate in on-call rotation
  • Participate in interdisciplinary team meetings
  • Provide nursing care services and case management to clients and families
  • Perform other duties as assigned

Requirements

What you’ll need
  • Degree in nursing field as a licensed practical nurse
  • 12 months facility/agency experience in nursing as an LPN preferred
  • Continuous daily travel required
  • Must have a valid driver’s license
  • Must have automobile insurance
  • Must be qualified to drive under the organization’s motor vehicle check
  • Good verbal and written communication, comprehension, computer, and interpersonal skills
  • Ability to speak, read, write, and understand English
  • Must have a current LPN license in good standing in the state(s) in which practicing
  • Must complete annual education requirements, including Compliance, Privacy, Security, and HIPAA training
  • Must adhere to organizational policies, procedures, regulations, and laws
