About the role

Key responsibilities & impact
  • Supervise, provide and direct physical therapy rehabilitation services under the general supervision of the Director of Clinical Management
  • Evaluate and treat residents/clients with injuries, illnesses or disabilities through therapeutic activities as ordered by physicians
  • Develop and evaluate plans of care in partnership with patients, representatives and caregivers
  • Provide patient and caregiver education and counseling
  • Communicate with physicians and other healthcare practitioners involved in plans of care
  • Supervise physical therapist assistants as applicable
  • Instruct, evaluate, develop plans for and supervise home health/hospice aides as applicable
  • Screen residents/clients according to facility/agency policy
  • Complete required documentation accurately and timely
  • Assist with marketing and program development as requested
  • Maintain productivity levels to meet facility/agency goals
  • Maintain current physical therapy knowledge through in-services and continuing education
  • Participate in interdisciplinary team and QAPI meetings
  • Assist with certification standards
  • Plan and complete staff in-services as requested
  • Perform other duties as assigned

Requirements

What you’ll need
  • Doctorate, Master or Bachelor of Science Degree in Physical Therapy
  • Two to four years experience preferred
  • Current license for physical therapy in the state in which practicing
  • Must have graduated from a PT program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE) or any successor organization
  • Frequent travel for Home and Community Services staff
  • Must have good communication, comprehension, and interpersonal skills
  • Ability to speak, read, write and understand English
  • Working knowledge of Windows, Microsoft Office Suite, and World Wide Web
  • Good written and verbal communication skills
  • Must adhere to all organizational policies, procedures, regulations and laws
  • Must complete annual education requirements including Compliance, Privacy, Security and HIPAA training
