About the role

Key responsibilities & impact
  • Approve care plans and monitor patient outcomes
  • Collaborate with clinicians to ensure care guidelines are met

  • Train, mentor, and supervise nursing staff
  • Assist with onboarding, scheduling, and performance management

  • Manage resource scheduling to optimize care delivery
  • Manage daily workflows and documentation in Home Care Home Base (HCHB)
  • Rotate on-call coverage to ensure continuous clinical support
  • Support and mentor field staff across multiple locations
  • Work alongside an interdisciplinary team

Requirements

What you’ll need
  • Active RN license in state of practice
  • 2+ years of home health leadership or clinical operations experience
  • Strong knowledge of home health regulations, reimbursement, and conditions of participation
  • Proficiency in Microsoft Office
  • Ability to learn new EMR systems quickly
  • Excellent communication, organizational, and problem-solving skills
  • Valid driver's license and reliable transportation
  • Ability to travel up to 25%
  • Home health certification (CHPN, CCM, or equivalent) preferred
