About the role

Key responsibilities & impact
  • Coordinate programs and activities for eligible individuals on a group basis
  • Liaise with community agencies and service providers, identify available services, and negotiate discounts where appropriate
  • Monitor and document participants’ use of community services
  • Develop and maintain a directory of state and local service providers
  • Organize educational events covering healthcare, agency support, life skills, and referral sources
  • Identify partners for health services and screenings
  • Help participants build informal support networks and engage volunteers
  • Conduct person-centered, non-clinical assessments, interventions, screenings, referrals, and monitoring
  • Outreach to eligible individuals, explain the program, and offer enrollment
  • Collaborate with care and service colleagues to achieve participant goals
  • Provide customer service and assist with basic needs and social determinants of health
  • Assist participants with community services including housekeeping, meals, transportation, personal services, financial assistance, adult day care, and counseling
  • Conduct telephonic and in-person home visits as required
  • Complete documentation, forms, care plans, follow-ups, monitoring, and performance reports
  • Participate in grants, special programs, interdisciplinary meetings, and required education
  • Maintain professional boundaries, manage time independently, respond to communications, and record labor time accurately
  • Uphold participant rights, dignity, confidentiality, compliance, and organizational core values

Requirements

What you’ll need
  • Bachelor’s degree in social service or related field
  • 3-5 years’ experience working with elderly and disabled populations
  • Good communication, comprehension and interpersonal skills
  • Working knowledge of Windows, Microsoft Office Suite and World Wide Web
  • Passing score on a computer literacy test
  • Ability to speak, read, write and understand English
  • Ability to perform basic arithmetic operations with money and use a calculator or Excel spreadsheet
  • Occasional travel or travel as directed by contract and department requests
  • Ability to comply with applicable federal, state, local regulations, laws, policies, procedures, HIPAA, and workplace safety requirements
