About the role

Key responsibilities & impact
  • Advocate for, organize, and problem-solve on behalf of residents
  • Coordinate programs and activities for residents
  • Serve as a liaison to community agencies and network with community service providers
  • Identify low-cost service providers and negotiate discounts
  • Engage residents to identify needs and make referrals to community agencies
  • Develop and maintain a Resource Directory of state and local service providers
  • Sponsor educational events covering health care, agency support, life skills, and referral sources
  • Provide customer service, address resident concerns, and assist with basic needs
  • Help residents access community services including housekeeping, meals, transportation, personal services, financial assistance, day care, and counseling
  • Monitor, document, and relay feedback or concerns from residents and families
  • Manage internal and external relationships using appropriate communication and interpersonal skills

Requirements

What you’ll need
  • Bachelor’s degree in social work or a related field preferred
  • Two or more years of experience delivering social services to elderly and/or family populations preferred
  • Working knowledge of supportive services and local community resources
  • Demonstrated ability to advocate, organize, problem-solve, and deliver measurable results for residents
