About the role
Key responsibilities & impact- Provide individualized, trauma-informed coaching and case management to approximately 18 families
- Support goals related to housing stability, education, employment, and family well-being
- Conduct assessments, develop service plans, complete IM+CANS updates, and maintain participant documentation
- Conduct regular in-person home visits
- Support families through housing transitions, eviction prevention, crisis situations, and other challenges
- Connect participants with community resources and coordinate services with internal and external partners
- Build participant engagement and community through monthly house meetings, activities, and supportive communication
- Participate in the Housing team’s rotating on-call schedule for after-hours emergencies
- Maintain IM+CANS certification and participate in the New Moms Coaching Model and professional development
- Participate in staff meetings, agency initiatives, and workgroups
- Supervise volunteers or interns as assigned
- Maintain timely communication and uphold New Moms’ values and service model
Requirements
What you’ll need- Bachelor’s in social work, psychology, counseling, or similar field; OR Bachelor’s in other field + 2 years clinical/human service experience; OR high school diploma + 5 years clinical or human service experience
- At least 1 year experience in a similar role preferred
- Valid driver’s license, daily access to a car, and insurance required
- Comfort using digital tools and case management software
- Ability to work one evening per week; occasional weekends
- Ability to sit/stand for up to 8 hours, walk stairs, and lift up to 30–40 lbs (or perform with reasonable accommodation)
- Maintain IM+CANS certification
- Ability to manage participant needs, deadlines, and documentation requirements
