About the role

Key responsibilities & impact
  • Deliver patient care in patient homes, hospitals, long-term care facilities, assisted living facilities, and retirement communities in the hospice service area
  • Collaborate with professional disciplines and the hospice interdisciplinary team to ensure effective patient care and desired outcomes
  • Provide on-call coverage after normal business hours, including acute symptom management and other patient needs
  • Admit patients to the hospice program and assess admission appropriateness according to regulatory requirements and hospice criteria
  • Complete admission, routine, death pronouncement, continuous care, and on-call visits
  • Perform comprehensive patient assessments and implement and evaluate interventions in the plan of care
  • Supervise hospice aides and LPNs according to regulations and agency policies
  • Educate and instruct patients and caregivers and promote a safe environment
  • Document patient care, charting, paperwork, variances, medication errors, adverse drug reactions, and infection surveillance
  • Coordinate care with patients, families, physicians, referral sources, hospice staff, and interdisciplinary team members
  • Respond to after-hours calls and pages from the hospice office, operator, or administrator on call
  • Offer and provide patient or family visits within required timeframes
  • Respond to new referrals received while on call
  • Participate in peer chart review, quality assessment/performance improvement activities, interdisciplinary meetings, staff meetings, and educational programs
  • Maintain clinical and regulatory expertise, complete required education and competencies, and comply with OSHA and safety policies

Requirements

What you’ll need
  • Georgia RN licensure
  • Current BLS certification
  • Associates Degree
  • Minimum of one (1) year of clinical nursing experience
  • Strong clinical nursing and assessment skills
  • Ability to understand, interpret and adhere to regulations, agency policy/procedure and accreditation standards
  • Excellent customer service and interpersonal skills
  • Skill in documenting clinical care in software system via laptop computer
  • Able to work independently and as a member of a team
  • Must have own automobile
  • Proof of valid driver's license
  • Current auto insurance with a minimum of $100,000 liability
  • Able to work a twenty-four (24) hour on-call assignment as scheduled
  • Must be able to lift up to 50 lbs frequently
  • Must be able to carry up to 20 lbs frequently
  • Must be able to read, write, reason, talk, keyboard, and drive
