About the role

Key responsibilities & impact

Conduct chronic disease care management, including depression and anxiety
Provide patient follow-up services and document patient outcomes
Educate patients and facilitate support groups
Mentor and coach less experienced Community Health Workers during certification training
Assess and address patient needs in person and by telephone
Coordinate patient care across health systems and navigate community resources
Advocate for patients and community members
Provide community health education and outreach to individuals, families, and local agencies
Assist with health fair events
Provide culturally aware social support and motivational support
Act as a health care coach, including self-management support and goal setting
Track and maintain a database of community outreach activities for grant tracking and reporting
Adapt communication and service explanations to specific patient populations
Maintain compliance with applicable laws, standards, and regulations
Report known or suspected unethical, safety, privacy, or compliance concerns

Requirements

What you’ll need

High School diploma or equivalent
Community Health Worker training certificate must be obtained within 1 year of hire
Must have computer skills and dexterity required for data entry and retrieval of information
Effective verbal and written communication skills and ability to present information clearly and professionally
Proficiency with Windows-style applications, role-specific software packages, and keyboard
Ability to communicate with individuals to educate and facilitate problem-solving
Strong interpersonal skills and ability to work with community members from diverse backgrounds
Ability to work independently
Organizational skills and ability to maintain a database
