About the role

Key responsibilities & impact
  • Conduct chronic disease care management, including depression and anxiety
  • Provide patient follow-up services, document outcomes, educate patients, and facilitate support groups
  • Mentor and coach less experienced Community Health Workers during certification training
  • Assess and address patient needs in person and by telephone
  • Coordinate patient care across health systems and navigate community resources
  • Advocate for patients and community members
  • Provide community health education and outreach to individuals, families, and local agencies
  • Assist with health fair events
  • Provide culturally aware social support and motivational support
  • Coach patients on self-management and goal setting
  • Track and maintain a database of community outreach activities for grant reporting
  • Adapt communication and service delivery to specific patient populations
  • Remain knowledgeable about applicable laws, accreditation standards, and regulatory requirements
  • Report known or suspected unethical, safety, privacy, or compliance concerns

Requirements

What you’ll need
  • High School diploma or equivalent
  • No work experience required
  • Community Health Worker training certificate must be obtained within 1 year of hire
  • Computer skills and dexterity for data entry and information retrieval
  • Effective verbal and written communication skills
  • Proficiency with Windows-style applications, role-specific software, and keyboard
  • Ability to educate individuals and facilitate problem-solving
  • Strong interpersonal skills and ability to work with diverse community members
  • Ability to work independently
  • Organizational skills and ability to maintain a database
