About the role
Key responsibilities & impact- Coordinate evaluation and preparation of patients for kidney transplantation and long-term post-transplant management
- Coordinate patient care across inpatient and outpatient settings throughout the transplant journey
- Conduct pre-transplant interviews and synthesize eligibility criteria for patients, families, physicians, and outside agencies
- Analyze pre-transplant clinical data and review medical and social histories
- Coordinate pre-transplant preparation and referral evaluations
- Educate patients and families about the transplant process, procedures, side effects, and complications
- Conduct post-discharge planning and education
- Provide direct care in the Outpatient Transplant Clinic and other inpatient/outpatient settings
- Assess post-transplant organ function and general health status and determine the need for additional intervention
- Collaborate with clinicians, community physicians, pharmacists, laboratories, home health agencies, insurers, and multidisciplinary transplant teams
- Utilize UNOS guidelines to select appropriate transplant candidates
- Provide telephone consultation and follow-up for patients and families
- Coordinate education, selection, and preparation for live kidney donors
- Provide rotational on-call medical triage and coordinate pre-transplant activities
- Develop and implement transplant care processes, policies, and standards
- Assist with onboarding and training new associates
- Monitor and input patient data to meet UNOS requirements and complete UNOS documentation
- Maintain relationships with external community stakeholders
- Participate in program development and research-related activities
Requirements
What you’ll need- Bachelor's Degree in Nursing (Required)
- RN - Registered Nurse - Ohio Board of Nursing
- Minimum 2 years of experience as a nurse in transplant or at least 5 years of clinical nursing experience
- Competence in managing renal and hepatic failure, diabetes, cardiopulmonary disease, hypertension, hyperlipidemia, and other transplantation-related disorders
- Competence in immunosuppression management, including adverse reactions and drug interactions
- Organizational, problem-solving, and decision-making skills
- Ability to prioritize clinical issues and case manage patients throughout the transplant process
- Ability to work in inpatient and outpatient settings
- Ability to provide rotational on-call coverage during off-shifts, weekends, and holidays
