About the role
Key responsibilities & impact- Provide direct clinical support and patient care at the LOA Osteoporosis Center
- Conduct patient histories and assessments, including symptoms, performance status scores, and clinical changes
- Review medications, distress or depression screening questionnaires, pain assessments, and vital signs
- Perform simple and complex diagnostic and therapeutic procedures
- Assess adherence to medications, treatments, and appointments
- Educate patients and families about diagnostic tests and treatment side effects
- Provide patient and caregiver education and educate nursing and other staff
- Document care-plan interventions and outcomes in medical records
- Evaluate prescription refill needs, medication effectiveness, and medication changes
- Provide equipment, resources, or community-service referrals to optimize patient comfort
- Manage and triage patient and family telephone calls, assess clinical problems, and develop related care plans
- Establish patient priorities and advise medical assistants, nurse practitioners, physicians, and other staff of patient needs
- Review laboratory results, imaging studies, and medical records before visits
- Advocate for patients regarding clinical changes and physical status
- Review laboratory results and inform patients about results, treatment, and scheduling
- Assist in developing clinical patient-management policies and procedures
- Contribute to problem lists covering physical, emotional, and psychosocial status
- Coordinate care with the Osteoporosis and clinical teams
- Travel among LOA offices in North Charleston, Mount Pleasant, and West Ashley
- Report to the LOA Osteoporosis Center
Requirements
What you’ll need- MA Certification or LPN with South Carolina license
- CPR Certification
- Experience in an outpatient setting and medical office experience preferred
- Knowledge of professional nursing theory, regulations, and practices to give and evaluate patient care
- Applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing patient care
- Ability to anticipate and react calmly to emergency situations, identify problems, and recommend solutions
- Experience preparing and maintaining records, writing reports, and responding to correspondence
- Experience in acute care, specific patient populations, and diseases
- Evidence of successful completion of didactic education and competency validation regarding safe handling, administration, patient education, and support
- Proficiency in clinical management of complex patients
- Ability to adapt quickly in a fast-paced clinic environment
- Current knowledge of clinical trends, skills, procedures, and protocols
- Knowledge and skills to provide care to patients of varying age groups, developmental stages, and cultural groups
- Knowledge of chronic illnesses and comorbidity management
- Written and verbal proficiency for communication with the interprofessional team
- Ability to coordinate patient care with interprofessional staff
- Flexibility and multitasking skills
- Leadership, priority-setting, and delegation skills
- Critical thinking and problem-solving skills
- Ability to use data, computer systems, and electronic health records
- Ability to work at three LOA office locations
- Experience in an outpatient setting and medical office experience is preferred, not required
