About the role

Key responsibilities & impact
  • Sell Option Care products and services to patients discharging from hospital care
  • Educate patients, families, facility staff, and referral sources about Option Care services and products
  • Maintain and grow referral-source relationships to increase sales and patient starts
  • Serve as a key Option Care contact and representative for referral sources
  • Interact daily with area service providers to sell services for discharging patients
  • Collaborate with clinical teams to determine patient suitability for alternative-site care
  • Communicate with agency staff, medical teams, patients, and families throughout discharge
  • Collect, review, complete, and secure signoff for preadmission information
  • Partner with Account Managers and Regional Sales Directors on area business and growth plans
  • Coordinate educational and promotional programs with Account Managers and marketing staff
  • Coordinate patient transitions to home or ATS and support therapy-start and patient-tracking data collection
  • Facilitate transfer of orders via phone, fax, and e-prescribing
  • Review medical records for medical history and insurance information
  • Maintain confidentiality of patient and proprietary information
  • Provide oversight and input to providers regarding patient transitions to home care
  • Coordinate communication with other providers
  • Arrange special medical supplies or appliances
  • Travel to partnership facilities, community hospitals, and medical practice offices

Requirements

What you’ll need
  • High School Diploma with 2 years of experience in applicable home infusion or post-acute infusion services, OR 1 year of similar experience with an associate’s degree, LPN
  • Experience establishing and maintaining relationships with individuals at all levels of the organization in the business community and with vendors
  • Experience applying knowledge of standard practices for all services offered and current relevant and applicable standards, including URAC and ACHC standards
  • Experience providing customer service to internal and external customers, including meeting quality standards and evaluating customer satisfaction
  • Basic PC skills, including internet, email, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint
  • Experience identifying operational issues and recommending and implementing strategies to resolve and improve processes
  • Reliable transportation for travel to care facilities, home visits, and multiple hospitals
  • Valid driver’s license and proof of insurance if using a personal vehicle
  • Ability to plan, organize, and make presentations
  • Experience growing service provider partnerships is preferred
