About the role

Key responsibilities & impact
  • Provide clinical education and training to ensure successful transition of patients to an Option Care Health care delivery model.
  • Partner with the sales team to increase the number of patients transitioned to Option Care Health care delivery.
  • Maintain and grow referral-source relationships to increase sales and patient starts.
  • Serve as a key point of contact and representative of Option Care Health for referral sources.
  • Educate patients, families, and facility staff about Option Care services and products.
  • Work with the clinical team to determine patient suitability for alternative site care.
  • Communicate with agency staff, medical teams, patients, and families throughout the discharge process.
  • Collect, review, and complete preadmission information and secure related signoff.
  • Partner with Account Manager and Regional Sales Director on area business and growth plans.
  • Coordinate educational and promotional programs with Account Manager and marketing staff.
  • Coordinate patient transitions to home or alternative treatment settings and facilitate order transfers.
  • Review medical records for pertinent history and insurance information.
  • Provide oversight and input regarding patient transitions to home care.
  • Coordinate with providers and arrange special medical supplies or appliances.
  • Travel to partner facilities, community hospitals, and medical practice offices.

Requirements

What you’ll need
  • High School Diploma with 2 years of experience in applicable home infusion or post-acute infusion services, OR 1 year of similar experience with an associate’s degree, LPN.
  • Experience establishing and maintaining relationships with individuals at all levels of the organization in the business community and with vendors.
  • Experience applying knowledge of standard practices for all services offered as well as current relevant and applicable standards (i.e. URAC, ACHC standards).
  • Experience providing customer service to internal and external customers, including meeting quality standards of services, and evaluation of customer satisfaction.
  • Basic PC skills: Able to competently use internet, email, Microsoft Word, Microsoft Excel, Microsoft PowerPoint.
  • Experience in identifying operational issues and recommending and implementing strategies to resolve and improve processes.
  • Access to a reliable means of transportation for travel to care facilities, home visits and multiple hospitals.
  • Valid driver’s license and proof of insurance required if using a personal vehicle.
  • Able to plan, organize and make presentations.
  • Preferred: Experience growing service provider partnerships.
