About the role

Key responsibilities & impact
  • Determine the suitability of individual patients for home care
  • Collect and organize patient-specific information for home-care suitability assessments and medication-related problem resolution
  • Design, recommend, monitor and evaluate patient-specific pharmacy care plans
  • Recommend modifications to prescribed medication therapy or monitoring parameters when appropriate
  • Complete nutritional assessments and make appropriate recommendations
  • Modify pharmacy care plans based on monitoring data and patient-specific information
  • Assess laboratory values and apply pharmacokinetic principles and formulas
  • Provide medication-use education and counseling to staff, professionals, patients and caregivers
  • Ensure continuity of pharmaceutical care between home and other care settings
  • Prepare, label, inspect and dispense medications according to policies, laws and USP standards
  • Calibrate, maintain, troubleshoot and advise on medication-administration and medication-related equipment
  • Assess, manage and report medication-related incidents and equipment errors or failures
  • Complete QAR documents and monitor controlled-substance inventory discrepancies
  • Respond to drug-information requests using analysis of the literature
  • Attend educational seminars or CEU opportunities and comply with state-mandated CEU requirements
  • Manage investigational drug products according to regulations and company policies
  • Utilize pharmacy support personnel, prioritize workload and organize workflow
  • Ensure accuracy of pharmacy support personnel's work
  • Comply with accreditation, legal, regulatory and safety requirements
  • Maintain confidentiality of patient and proprietary information
  • May perform other duties as assigned

Requirements

What you’ll need
  • Graduate of an accredited School of Pharmacy
  • Current, active Pharmacy license as granted by the State Board of Pharmacy
  • One of the following: at least one year of experience in sterile IV admixture procedures; knowledge of non-sterile compounding, immunizations and medication therapy management (MTM); completion of an ASHP accredited Pharmacy Residency Program; completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy; or equivalent experience approved by the VP of Pharmacy and/or SVP of Clinical Services
  • Experience providing customer service to internal and external customers, including meeting quality standards and evaluating customer satisfaction
  • Basic-level skill in Microsoft Word
  • Basic-level skill in Microsoft Excel
  • Basic-level skill in Microsoft PowerPoint
  • Willingness to travel up to 5% of the time for business purposes within the state
  • Preferred: completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience
  • Preferred: experience in home care, hospital or other clean room setting
  • At least 1 year of experience in indirect management of team members, including assisting in development, training and assignment of work/projects to other team members
