About the role

Key responsibilities & impact
  • Determine individual patients’ suitability for home care
  • Collect and organize patient-specific information for home-care assessments and medication-related decisions
  • Design, recommend, monitor and evaluate patient-specific pharmacy care plans
  • Recommend modifications to medication therapy or monitoring parameters when appropriate
  • Complete nutritional assessments and make recommendations
  • Modify pharmacy care plans based on monitoring data and patient-specific information
  • Assess laboratory values and apply pharmacokinetic principles and formulas
  • Provide medication-use education and counseling to staff, professionals, patients and caregivers
  • Ensure continuity of pharmaceutical care between home and other patient-care settings
  • Prepare and dispense medications using appropriate techniques and organizational policies
  • Calibrate, maintain, troubleshoot and advise on medication-administration and medication-related equipment
  • Assess, manage and report medication-related incidents and equipment errors or failures
  • Provide drug-information responses based on literature analysis
  • Attend educational seminars and participate in continuing education opportunities
  • Manage investigational drug products according to regulations and policies
  • Effectively utilize pharmacy support personnel and organize workflow
  • Comply with accreditation, legal, regulatory, safety, confidentiality and ethical requirements
  • Maintain perpetual inventory systems for controlled substances and report discrepancies
  • Perform other duties as assigned

Requirements

What you’ll need
  • Graduate of an accredited School of Pharmacy
  • Current, active Pharmacy license as granted by the State Board of Pharmacy
  • One of the following: at least 1 year of experience in sterile IV admixture procedures; knowledge of non-sterile compounding, immunizations and medication therapy management (MTM); completion of an ASHP accredited Pharmacy Residency Program; completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy; or equivalent experience approved by the VP of Pharmacy and/or SVP of Clinical Services
  • Experience providing customer service to internal and external customers, including meeting quality standards and evaluating customer satisfaction
  • Basic Microsoft Word skills
  • Basic Microsoft Excel skills
  • Basic Microsoft PowerPoint skills
  • Willingness to travel up to 5% of the time for business purposes within the state
  • At least 1 year of experience in indirect management of team members, including assisting in development, training and assignment of work/projects
  • Experience in home care, hospital or other clean room setting preferred
  • Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience preferred
