Home Infusion Nurse

About the role
Key responsibilities & impact- Initiate, develop, and implement intermediate nursing plans of care and treatments
- Evaluate patient progress toward goals and modify treatment plans based on patient status or physician orders
- Organize and participate in direct patient care, perform treatments, administer medications, and educate patients and families/caregivers
- Assess patient needs and physical status during skilled visits
- Obtain and clarify physician orders and promptly inform physicians of significant patient-condition changes
- Provide training and mentorship to Infusion Nurse I staff as needed
- Teach patients and families/caregivers to achieve independence with prescribed therapy and care needs
- Communicate with clinical teams, medical providers, patients, and families/caregivers to facilitate continuity of care
- Prepare real-time clinical documentation and submit it within established timelines
- Coordinate discharge planning and prepare discharge summaries
- Comply with agency operations, company policies, professional standards, regulations, and accreditation standards
- Use computers and automation technologies to validate treatment orders, document care, and follow assigned visit schedules
- Respond to patient requests and initiate emergency procedures as necessary
- Safeguard confidential patient and proprietary information in accordance with HIPAA
- Explain Option Care Health services and related financial obligations to patients
- Complete required training, in-services, and continuing education
- Provide oversight and coordination of paraprofessionals in the home setting
- Participate in multidisciplinary conferences, precepting, training, mentoring, orientation, onboarding, and supervisory activities as assigned
- Participate in nursing department on-call responsibilities
- Insert PICC/Midlines per physician order when applicable
- Troubleshoot and declot obstructed central line catheters when applicable
- Monitor PICC/Midlines and educate patients on infection prevention and routine care when applicable
Requirements
What you’ll need- Active and unrestricted RN license in the state of practice
- Minimum of 1 years of nursing experience
- Current CPR certification required
- Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs
- Advanced certification and training as applicable
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel with proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to apply knowledge in the effective use of nursing technology tools to communicate and document care provided
- Ability to lift up to 50 pounds, with a maximum lifting of 75 pounds
- Willing to travel up to 100% of the time for business purposes
- Bachelor of Science in Nursing from an accredited school of nursing preferred
- 1-2 years of previous infusion nursing experience preferred
