About the role
Key responsibilities & impact- Coordinate and provide intermediate direct patient care in patients’ homes or alternate infusion suites
- Initiate, develop, implement, and evaluate intermediate nursing plans of care
- Perform treatments, administer medications, and educate patients and families/caregivers
- Assess patient needs and physical status during skilled visits
- Obtain and clarify physician orders and report significant changes in patient condition
- Provide training and mentorship to Infusion Nurse I team members as needed
- Teach patients and families to achieve independence with prescribed therapy and care needs
- Communicate with clinical teams, medical providers, patients, and caregivers to facilitate continuity of care
- Prepare real-time clinical documentation and submit nursing documentation for billing compliance
- Coordinate discharge planning and prepare discharge summaries
- Comply with agency policies, professional standards, regulations, and accreditation standards
- Use computers and automation technologies to validate treatment orders and document care
- Respond to patient requests and initiate emergency procedures when necessary
- Safeguard patient and proprietary information in accordance with HIPAA
- Explain Option Care services and related financial obligations to patients
- Complete training, in-services, and continuing education
- Provide oversight and coordination of paraprofessionals in home settings
- Participate in multidisciplinary conferences, precepting, training, mentoring, and nursing department on-call responsibilities
- Insert PICC/Midlines and troubleshoot or declot obstructed central line catheters, if applicable
Requirements
What you’ll need- Active and unrestricted RN license in the state of practice
- Minimum of 1 years of nursing experience
- Current CPR certification required
- Demonstrated competency in patient care standards required for safe delivery of services and applicable infusion skill sets
- Advanced certification and training as applicable
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel with proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to lift up to 50 pounds, with a maximum lifting requirement of 75 pounds
- Willing to travel up to 100% of the time for business purposes
- Bachelor of Science in Nursing from an accredited school of nursing preferred
- 1–2 years of previous infusion nursing experience preferred
