About the role
Key responsibilities & impact- Develop, coordinate, and provide direct infusion patient care in patients’ homes or alternate infusion suites
- Initiate, develop, implement, and evaluate advanced and complex nursing plans of treatment
- Perform treatments, administer medications, and educate patients and families/caregivers
- Assess patient needs and physical status during skilled visits
- Obtain and clarify physician orders and report significant changes in patient condition
- Teach patients and families to achieve independence with prescribed therapy and care needs
- Provide training and mentorship to lower-level nurses
- Participate in process improvement programs and chart audits
- Research and recommend changes aligned with nursing best practices
- Communicate with clinical teams, medical providers, patients, and caregivers to ensure continuity of care
- Prepare real-time clinical documentation and submit it for billing compliance
- Coordinate discharge planning and prepare discharge summaries
- Comply with agency policies, professional standards, regulations, and accreditation standards
- Use computers and automation technologies to validate orders, document care, and follow visit schedules
- Respond to patient requests and initiate emergency procedures when necessary
- Safeguard confidential patient and company information in accordance with HIPAA
- Explain Option Care services and related financial obligations to patients
- Complete training, in-services, and continuing education
- Provide oversight and coordination of paraprofessionals
- Participate in multidisciplinary conferences, precepting, onboarding, and mentoring
- Participate in nursing department on-call responsibilities
- Insert PICC/Midlines, troubleshoot or declot obstructed central lines, and educate on line care when applicable
Requirements
What you’ll need- Active and unrestricted RN license in the state of practice
- CRNI required
- BSN and at least 5 years of nursing experience, or at least 7 years of nursing experience with 3 years of infusion experience
- Current CPR certification required
- Demonstrated competency in patient care standards and infusion skill sets
- Advanced certification and training as applicable
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel with proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to lift up to 50 pounds, with a maximum lifting of 75 pounds
- Willingness to travel up to 100% of the time for business purposes
- Occupational exposure to blood, body fluids, hazardous substances, needles, chemicals, and unpredictable home environments
- Safety equipment requirements including closed-toe shoes, facemask, goggles, gown and/or gloves, and a functioning communication device
