About the role

Key responsibilities & impact
  • Coordinate and provide intermediate direct patient care to patients in the home or an alternate infusion suite
  • Initiate, develop, and implement intermediate nursing plans of care and evaluate patient progress
  • Organize and participate in direct patient care, perform treatments, administer medications, and educate patients and families/caregivers
  • Modify treatment plans in response to changing patient status or physician orders
  • Assess patient needs and physical status during skilled visits and reassess response to therapy
  • Obtain and clarify physician orders and inform physicians of significant patient condition changes
  • Provide training and mentorship to Infusion Nurse I staff as needed
  • Teach patients and families using a patient-centered approach to support independence with prescribed therapy
  • Communicate with clinical teams, medical providers, patients, and caregivers to ensure continuity of care
  • Prepare real-time clinical documentation and submit it within established timelines
  • Coordinate discharge planning and prepare discharge summaries
  • Comply with agency operations, company policies, professional standards, regulations, and accreditation standards
  • Use computers and automation technologies to validate treatment orders, document care, and follow assigned visit schedules
  • Respond to patient requests and initiate emergency procedures when necessary
  • Safeguard confidential patient and proprietary information in accordance with HIPAA
  • Explain the company’s services and related financial obligations to patients
  • Complete required training, in-services, and continuing education
  • Provide oversight and coordination of paraprofessionals and act as Supervisor when applicable
  • Participate in multidisciplinary conferences, precepting, training, mentoring, and nursing department on-call responsibilities
  • Insert PICC/Midlines per physician order when applicable
  • Troubleshoot and declot obstructed central line catheters when applicable
  • Monitor PICC/Midlines and educate patients on infection prevention and routine care when applicable
  • Perform other related duties as directed by the supervisor

Requirements

What you’ll need
  • Active and unrestricted RN license in the state of practice
  • Minimum of 1 years of nursing experience
  • Current CPR certification required
  • Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs
  • Advanced certification and training as applicable
  • Required licensure to operate a motor vehicle in the state of practice
  • Access to a vehicle for business travel with proof of liability insurance
  • Basic knowledge of computer operating systems and software applications
  • Ability to lift up to 50 pounds, with a maximum lifting of 75 pounds
  • Willing to travel up to 100% of the time for business purposes
  • Bachelor of Science in Nursing from an accredited school of nursing preferred
  • 1-2 years of previous infusion nursing experience preferred
