About the role
Key responsibilities & impact- Coordinate and provide intermediate direct patient care in patients’ homes or alternate infusion suites under a physician’s plan of care
- Initiate, develop, implement, and evaluate intermediate nursing plans of care and treatments
- Perform treatments, administer medications, educate patients and families/caregivers, and modify treatment plans as needed
- Assess patient needs and physical status during skilled visits and communicate significant changes to physicians
- Provide training and mentorship to Infusion Nurse I’s as needed
- Teach patients and families using a patient-centered care approach to support independence with prescribed therapy
- Communicate with clinical teams, medical providers, patients, and caregivers to facilitate continuity of care
- Prepare and submit real-time clinical documentation in compliance with billing and policy requirements
- Coordinate discharge planning and prepare discharge summaries
- Comply with agency operations, company policies, professional standards, regulations, and accreditation standards
- Use computers and automation technologies to validate treatment orders, document care, and follow assigned visit schedules
- Respond to patient requests and initiate emergency procedures when necessary
- Safeguard confidential patient and proprietary information in accordance with HIPAA
- Explain company services and related financial obligations to patients
- Complete required training, in-services, and continuing education
- Provide oversight and coordination of paraprofessionals and act as Supervisor when applicable
- Participate in multidisciplinary conferences, precepting, training, mentoring, and nursing department on-call responsibilities
- Insert PICC/Midlines per physician order, troubleshoot/declot obstructed central line catheters, and monitor PICC/Midline care when applicable
- Perform other related duties as directed
Requirements
What you’ll need- Active and unrestricted RN license in the state of practice
- Minimum of 1 years of nursing experience
- Current CPR certification required
- Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs
- Advanced certification and training as applicable
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel with proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to lift up to 50 pounds, with a maximum lifting of 75 pounds
- Willing to travel up to 100% of the time for business purposes
- Bachelor's of Science in Nursing from an accredited school of nursing preferred
- 1-2 years of previous infusion nursing experience preferred
