About the role

Key responsibilities & impact
  • Initiate, develop, and implement intermediate nursing plans of care and treatments
  • Evaluate patient progress toward care goals and modify treatment plans in response to patient status or physician orders
  • Provide direct patient care in patients’ homes or alternate infusion suites
  • Perform treatments and administer medications
  • Educate patients and families/caregivers using a patient-centered approach
  • Assess patient needs and physical status during skilled visits
  • Obtain and clarify physician orders and promptly inform physicians of significant changes
  • Provide training and mentorship to Infusion Nurse I team members as needed
  • Communicate with clinical teams, medical providers, patients, and families to facilitate continuity of care
  • Prepare real-time clinical documentation and submit nursing documentation within established timelines
  • Coordinate discharge planning and prepare discharge summaries
  • Comply with agency operations, company policies, professional standards, regulations, and accreditation standards
  • Use computers and automation technologies to validate treatment orders, communicate care, and follow assigned visit schedules
  • Respond to patient requests and initiate emergency procedures as necessary
  • Safeguard confidential patient and proprietary information, including HIPAA compliance
  • Explain the company’s services and related financial obligations to patients
  • Complete required training, in-services, and continuing education
  • Provide oversight and coordination of paraprofessionals in the home setting
  • Participate in multidisciplinary team conferences and precept, train, and mentor other nurses
  • Participate in nursing department on-call responsibilities
  • Insert PICC/Midlines per physician order, if applicable
  • Troubleshoot and declot obstructed central line catheters, if applicable
  • Monitor PICC/Midlines and educate patients on line infection prevention and routine care, if applicable
  • Perform other related duties as directed by the supervisor

Requirements

What you’ll need
  • Active and unrestricted RN license in the state of practice
  • Minimum of 1 years of nursing experience
  • Current CPR certification required
  • Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs
  • Advanced certification and training as applicable
  • Required licensure to operate a motor vehicle in the state of practice
  • Access to a vehicle for business travel with proof of liability insurance
  • Basic knowledge of computer operating systems and software applications
  • Ability to apply knowledge in the effective use of nursing technology tools to communicate and document care provided
  • Ability to lift up to 50 pounds, with a maximum lifting of 75 pounds
  • Willing to travel up to 100% of the time for business purposes
  • Bachelor of Science in Nursing from an accredited school of nursing preferred
  • 1–2 years of previous infusion nursing experience preferred
