About the role

Key responsibilities & impact
  • Provide patient-centered Medication Therapy Management services to optimize medication therapy and improve clinical, economic, and humanistic outcomes
  • Deliver Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, Cost-Effective Interventions, and Transitions in Care
  • Conduct proactive patient outreach to explain MTM program benefits and engage hesitant patients
  • Receive and respond to incoming patient calls and take appropriate action based on protocols
  • Accurately document all patient discussions
  • Communicate and document clinical recommendations to prescribers and interdisciplinary care teams
  • Provide patient-centered education on chronic disease states and preventive and therapeutic medication regimens
  • Navigate MTM platforms and multiple clinical databases or applications
  • Produce accurate, concise, and timely clinical documentation
  • Perform MTM services telephonically or through remote video link (telehealth)
  • Troubleshoot basic technical issues involving internet connectivity, software navigation, and peripheral equipment

Requirements

What you’ll need
  • Minimum 1 year of work experience
  • Bachelor's Degree in Pharmacy or PharmD from a college accredited by ACPE, or graduate of a foreign college of pharmacy
  • Current license to practice Pharmacy in the state(s) you are practicing in
  • Ability to acquire pharmacist licensure in multiple states, if requested
  • Completion of an MTM certificate course
  • Maintain active pharmacist licensure in required state(s) based on business needs
  • Adhere to state pharmacy laws, CMS program requirements, HIPAA standards, and internal privacy and compliance guidelines
  • Skilled in assessing medication therapy for appropriateness, effectiveness, safety, dosing accuracy, unnecessary or missing therapies, adverse drug reactions, and adherence concerns
  • Proficient in developing evidence-based interventions
  • Knowledge of cost-effective medication strategies
  • Experience identifying adherence barriers and creating patient-specific solutions
  • Clinical therapeutic expertise for adult and geriatric populations
  • Proficient with Microsoft Office applications, including Excel, Word, and Outlook
  • Ability to troubleshoot basic technical issues
  • Ability to work independently in a collaborative environment
  • Preferred: Board Certification in relevant specialty
  • Preferred: Completion of an ASHP-accredited pharmacy residency and/or fellowship
  • Preferred: Pharmacist licensure in multiple states
  • Bilingual and fluent in English and Spanish/Mandarin/another language highly preferred
  • Motivational Interviewing Skills preferred
