About the role
Key responsibilities & impact- Provide direct nursing care to residents in a skilled nursing and rehabilitation facility
- Supervise day-to-day nursing activities performed by nursing assistants
- Ensure compliance with federal, state, local, and facility policies, regulations, and quality standards
- Administer medications, treatments, catheterization, tube feedings, suction, wound dressings, specimens, vital signs, and other nursing procedures
- Review and maintain resident charts, care plans, medication records, orders, and documentation
- Admit, transfer, and discharge residents; assist with discharge planning and transportation
- Monitor resident conditions and notify physicians and next of kin of changes, accidents, or incidents
- Coordinate nursing services with physicians, rehabilitation, social, activities, dietary, and other departments
- Develop work assignments, provide shift leadership, conduct rounds, and address staffing needs
- Participate in performance evaluations, quality assurance, facility surveys, infection control, safety, and disaster preparedness
- Train, orient, and support nursing personnel and participate in in-service and continuing education programs
- Maintain medication, equipment, supply, sanitation, confidentiality, and information-security standards
- Review resident care plans and support MDS, RAPs, and triggers
- Prepare reports, census records, incident reports, medical forms, and other administrative documentation
Requirements
What you’ll need- Nursing Degree from an accredited college or university, or graduation from an approved LPN/LVN program
- Current, unencumbered, active South Carolina LPN/LVN license
- Current CPR certification
- Must maintain required continuing education/licensing
- Must remain in good standing with the State Board of Nursing
- Ability to read, write, speak, and understand English
- Ability to work 12-hour shifts and weekends
- Experience in a fast-paced, high-admissions healthcare setting preferred
- Knowledge and experience with PCC preferred
- Ability to provide age-appropriate resident care
- Knowledge of nursing and medical practices, procedures, and applicable laws and regulations
- Leadership and supervisory ability
- Ability to make independent decisions and work effectively with residents, personnel, and support agencies
- Ability to move intermittently and lift, push, pull, or carry at least 25 pounds
- Must meet facility general health requirements, including medical and physical examination
