About the role
Key responsibilities & impact- Provide direct nursing care to residents and supervise day-to-day nursing assistant activities
- Provide skilled nursing care and administer medications and treatments
- Monitor residents’ health and communicate condition changes to the healthcare team, physicians, and families
- Collaborate with physicians, therapists, CNAs, nurses, social services, activities, and other departments
- Admit, transfer, and discharge residents
- Complete nursing documentation, charting, reports, forms, census reports, and incident reports
- Transcribe and manage physician orders, treatment plans, medication cards, and care plans
- Perform nursing procedures including catheterization, tube feedings, suction, dressing changes, colostomy and drainage-bag care, specimen collection, vital signs, and range-of-motion exercises
- Review and implement resident care plans and coordinate MDS/RAPs/Triggers activities
- Conduct rounds, supervise staff assignments, evaluate personnel performance, and report staffing needs
- Maintain medication, medical supply, equipment, safety, sanitation, and infection-control standards
- Participate in quality assurance programs, facility surveys, discharge planning, and staff training
- Maintain resident confidentiality and information-system security
- Attend continuing education and required in-service training
Requirements
What you’ll need- Current South Carolina LPN license in good standing
- Nursing Degree from an accredited college or university, or graduation from an approved LPN/LVN program
- Current, unencumbered, active license to practice as an LPN/LVN in South Carolina
- Current CPR certification
- Must maintain all required continuing education/licensing
- Must remain in good standing with the State Board of Nursing
- Must be able to read, write, speak, and understand English
- Ability to read technical procedures and policy/procedure manuals
- Knowledge and experience with PCC preferred
- Ability to apply fractions, percentages, ratios, and proportions
- Knowledge and skills to provide age-appropriate resident care
- Ability to make independent decisions and supervise personnel
- Knowledge of nursing and medical practices, laws, regulations, and nursing facility guidelines
- Leadership and supervisory ability
- Ability to plan, organize, develop, implement, and interpret care programs and policies
- Must meet facility general health requirements, including medical and physical examination
- Ability to move intermittently and push, pull, move, and/or carry at least 25 pounds 50 feet
- May need to assist with resident evacuation during emergencies
