About the role
Key responsibilities & impact- Provide direct nursing care to residents in a skilled nursing and rehabilitation facility
- Supervise day-to-day nursing activities performed by nursing assistants
- Administer medications, treatments, catheterization, tube feedings, suction, dressings, specimens, vital signs, and other nursing services
- Monitor residents’ conditions and report changes to physicians, nurse supervisors, and families as appropriate
- Document care, orders, incidents, admissions, transfers, discharges, and resident records
- Coordinate with physicians, therapists, CNAs, social services, dietary services, and interdisciplinary teams
- Admit, transfer, and discharge residents and assist with discharge planning and transportation
- Review and maintain resident care plans, assessments, MDS processes, and restorative programs
- Lead, schedule, train, evaluate, and support nursing personnel and CNAs
- Participate in quality assurance, facility surveys, infection control, safety, and emergency preparedness programs
- Maintain medication, equipment, supply, sanitation, confidentiality, HIPAA, and regulatory compliance
- Participate in staff orientation, in-service training, continuing education, and professional development
- Prepare reports, recommendations, census records, charge slips, and administrative documentation
Requirements
What you’ll need- Current South Carolina LPN license
- Nursing Degree from an accredited college or university, or graduation from an approved LPN/LVN program
- Current, unencumbered, active license to practice as an LPN/LVN in South Carolina
- Current CPR certification
- Must be able to read, write, speak, and understand the English language
- Must maintain all required continuing education/licensing
- Must remain in good standing with the State Board of Nursing at all times
- Knowledgeable of nursing and medical practices and procedures, and laws, regulations, and guidelines pertaining to nursing care facilities
- Leadership and supervisory ability
- Ability to provide age-appropriate resident care
- Ability to make independent decisions when circumstances warrant
- Ability to plan, organize, develop, implement, and interpret nursing care programs, goals, policies, and procedures
- Must meet the facility’s general health requirements, including medical and physical examination
- Ability to move, push, pull, and/or carry at least 25 pounds to a minimum height of 5 feet and distance of 50 feet
- Skilled nursing or long-term care experience is a plus
- Knowledge and experience with PCC preferred
