About the role

Key responsibilities & impact
  • Provide oversight on facilities’ weekly PDPM meetings for skilled patients
  • Track and monitor effective utilization of PDPM tools, including Physician Diagnosis Verification, Admission/Discharge CSR(GG), Weekly CSR, PDPM Weekly Tracking Tool, and Point Right
  • Create clinical reporting systems between RDO and RDCS
  • Hold monthly regional MDS workshops on PDPM and CMS-related regulations
  • Track and monitor QM and QASP performance and provide data analysis and clinical recommendations
  • Assist with MDS/PDPM claims review, accuracy checks, and trend analysis
  • Provide PDPM and MDS training to new MDS Coordinators
  • Develop performance evaluation systems for MDS Nurses with facility Administrators
  • Ensure resident assessment protocol triggers are included in care plans
  • Assess health status and level of care for new admissions
  • Ensure accurate and timely completion, scheduling, data entry, correction, verification, and submission of MDS assessments
  • Coordinate interdisciplinary participation in MDS completion and resident care conferences
  • Assist with formulating and revising individualized care plans
  • Evaluate care plans and resident outcomes; ensure reassessment and revision as appropriate
  • Manage facility level-of-care changes and notify departments
  • Generate and transmit required acuity and level-of-care forms
  • Maintain confidentiality of resident care information and protected health information
  • Provide regional support, training, and mentorship to MDS Coordinators
  • Provide on-site support 4 days/week and remote support 1 day/week

Requirements

What you’ll need
  • RN license in good standing in CA
  • MDS certified - RAC-CT
  • Recent experience with MDS 3.0
  • Recent experience in skilled nursing facilities
  • Ability to provide on-site regional support 4 days/week and remote support 1 day/week
