About the role
Key responsibilities & impact- Coordinate and complete MDS assessments accurately and timely
- Collaborate with nursing, therapy, dietary, and social services teams
- Ensure compliance with Medicare, Medicaid, and regulatory requirements
- Monitor assessment schedules and care planning processes
- Assist in maximizing reimbursement while maintaining quality resident care
- Review clinical documentation for accuracy and completeness
- Advocate for residents by ensuring assessments reflect their care needs
- Oversee facilities’ weekly PDPM meetings for skilled patients
- Track and monitor utilization of PDPM tools
- Track and monitor QM and QASP performance and provide data analysis and clinical recommendations
- Provide PDPM and MDS training to new MDS Coordinators
- Assess health status and level of care of new admissions
- Coordinate interdisciplinary participation in MDS completion
- Maintain MDS assessment schedules and reference dates
- Perform MDS data entry, electronic submission, corrections, and record maintenance
- Schedule and conduct resident care conferences
- Assist with formulating and revising individualized care plans
- Evaluate care plans and resident outcomes
- Manage facility level-of-care changes and notify departments
- Generate and transmit required acuity and level-of-care forms
- Maintain confidentiality of resident care information and protected health information
- Supervise staff as required
Requirements
What you’ll need- Current, active RN license
- Previous MDS experience required
- Strong understanding of Medicare, Medicaid, and reimbursement processes
- Knowledge of RAI guidelines and care planning
- Excellent organizational and communication skills
- Ability to work independently and as part of a team
- Long-term care/skilled nursing experience preferred
- Graduation from an Accredited School of Nursing
- Must be in good standing with the State Board of Nursing
- Maintain all required continuing education/licensing requirements
- Proficiency in Microsoft Suite products
- Knowledge and experience with PCC preferred
- 3 years of supervisory experience preferred
