About the role

Key responsibilities & impact
  • Oversee nursing operations during the weekend shift
  • Support and guide nursing staff in providing resident care
  • Ensure compliance with policies, procedures, and regulations
  • Respond to resident, family, and staff needs
  • Collaborate with interdisciplinary teams
  • Maintain accurate documentation and reports
  • Communicate with residents and families about plans of care, PT, OT, and treatment protocols
  • Participate in morning meetings/stand-up meetings
  • Answer questions about care, treatment plans, illness progression, and related issues
  • Monitor and adjust resident statuses based on changing needs and conditions
  • Organize and prioritize work for new, current, and discharging residents
  • Perform utilization review activities
  • Coordinate care with residents, providers, facility financial services, and third-party payers
  • Oversee admissions and discharge activities
  • Coordinate referrals to and from the facility
  • Complete charting and required paperwork

Requirements

What you’ll need
  • Current Registered Nurse (RN) license in good standing, or valid LVN/LPN license in the state of employment
  • Bachelor’s Degree in Nursing or Social Work
  • Two (2) year clinical nursing experience preferred
  • Supervisory or leadership experience preferred
  • Long-term care / nursing home experience a plus
  • Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits
  • PCC knowledge
  • Intermediate computer skills
  • Strong communication, critical thinking, time management, and organizational skills
  • Ability to read technical procedures and policy and procedure manuals
  • Ability to solve practical problems and interpret written, oral, diagram, or schedule instructions
  • Must maintain required continuing education/licensing
  • Must remain in good standing with the Department of Public Health
  • Certificate as a certified Case Manager (CCM) a plus
  • Ability to lift up to 50 pounds frequently and perform required physical activities
