About the role

Key responsibilities & impact
  • Assess and monitor residents' wounds
  • Provide and oversee appropriate wound treatments
  • Develop and update individualized wound care plans
  • Monitor healing progress and identify changes in condition
  • Communicate with physicians, nursing staff, residents, families, and interdisciplinary healthcare teams
  • Maintain accurate and descriptive wound care documentation and nurses’ notes
  • Carry out physicians’ orders, including medication and treatment administration
  • Perform treatments while protecting residents’ dignity and self-respect
  • Notify physicians of delayed healing, deterioration, or infection signs and symptoms
  • Ensure wound care management services, including special mattresses and nutritional supplements, are included in care plans
  • Assist with resident care plans and physician/provider visits and examinations
  • Notify families and attending physicians of significant treatment-related issues
  • Participate in nursing staff orientation and ongoing training
  • Report significant findings, changes in condition, and concerns to the RN Supervisor and/or Director of Nursing
  • Participate in facility surveys and inspections when requested
  • Assist with overall supervision of nursing staff

Requirements

What you’ll need
  • Active, unrestricted LPN license required
  • Previous wound care experience required
  • Hands-on experience with wound assessment and treatment
  • Strong clinical and documentation skills
  • Possession of a Nursing Degree from an accredited college or university
  • Two (2) years or more experience, preferably in a long-term care facility
  • Current CPR certification
  • Must maintain all required continuing education for licensing
  • Must remain in good standing with the State Board of Nursing
  • Ability to read technical procedures and policy and procedure manuals
  • Ability to effectively present information and respond to questions from managers and employees
  • Ability to apply fractions, percentages, ratios and proportions to practical situations
  • Ability to solve practical problems and interpret written, oral, diagram, or schedule instructions
  • Knowledge and experience with PCC preferred
  • Ability to lift up to 50 pounds frequently
  • Ability to perform frequent standing/walking, reaching, pushing/pulling, talking/hearing, tasting/smelling, and occasional climbing, balancing, stooping, kneeling, crouching or crawling
