About the role
Key responsibilities & impact- Assess, manage, and treat wounds, pressure injuries, skin tears, ulcers, and other dermatologic conditions
- Complete comprehensive skin assessments and quarterly dermatologic evaluations
- Develop individualized treatment plans with physicians, Medical Director, Director of Nursing, therapy, dietary, and the Interdisciplinary Care Plan Team
- Provide written and verbal reports on resident progress and wound healing
- Monitor wound healing, update care plans, and recommend treatment modifications
- Ensure pressure injury prevention interventions, including repositioning, nutrition support, skin protection, special mattresses, and nutritional supplements
- Provide direct nursing care, medications, and treatments according to physician orders
- Educate residents, families, and staff on wound prevention and treatment plans
- Maintain accurate nursing notes and documentation in compliance with regulations and HIPAA
- Monitor wound care supplies and equipment inventory
- Report significant findings, resident concerns, suspected abuse, safety issues, exposures, and security concerns
- Assist physicians and healthcare providers with visits and examinations
- Participate in nursing staff orientation, ongoing training, and facility inspections
- Assist with overall supervision of nursing staff
- Report to and work under the Director of Nursing
Requirements
What you’ll need- Experienced RN or LPN Wound Nurse; this is not an entry-level position
- Previous wound care experience required
- Current, active, unrestricted RN or LPN license in good standing
- Minimum of 2 years of nursing experience
- Possession of a Nursing Degree from an accredited college or university
- Current CPR certification
- Ability to read technical procedures and policy and procedure manuals
- Excellent assessment, documentation, communication, and customer service skills
- Strong clinical judgment and critical thinking
- Ability to work independently while collaborating with an interdisciplinary team
- Knowledge and experience with PCC preferred
- Must maintain required continuing education for licensing and remain in good standing with the State Board of Nursing
- Ability to lift up to 50 pounds frequently
- Ability to meet stated standing, walking, reaching, pushing/pulling, and other physical demands
