About the role

Key responsibilities & impact

Develop and execute targeted marketing campaigns to attract new clients and improve retention
Build relationships with contracted vendors and providers to ensure high-quality service and care through regular patient interactions
Engage with the community to promote services and identify individuals with potential care coordination or case management needs
Support referrals and align outreach efforts with organizational goals
Monitor market trends and performance metrics
Meet with patients and families to discuss Pathfinder home health services
Act as liaison between referral sources, patients, families, and the office
Submit required documents to vendors for credentialing to enter facilities
Devise marketing campaigns to increase client base and referrals
Plan, organize, and execute marketing programs
Attend trade shows, events, and mandatory company meetings
Manage individual marketing budget
Communicate with Intake to verify receipt of referrals
Review the Intake log to follow pending referrals and admission or non-admit decisions
Keep abreast of health industry trends, competition, and new opportunities

Requirements

What you’ll need

A proven track record in the Home Health industry
A desire to provide the highest level of customer service
Ability to persuade referral sources by presenting the benefits of Pathfinder
Strong verbal and written communication skills
High School Diploma or equivalent
