About the role

Key responsibilities & impact
  • Serve as the vital link between referral sources, patients, and the clinical team
  • Respond to patient and referral source requests and concerns
  • Build partnerships with hospitals, physicians, and community organizations through face-to-face meetings
  • Educate referral sources on Alpha’s services
  • Maintain positive relationships with current referral sources
  • Coordinate smooth, timely admissions for patients transitioning to home health
  • Represent the agency in transitional care activities and strategic relationships with health systems, hospitals, inpatient facilities, associations, and physician groups
  • Identify, contact, and manage potential clients, patients, and referral sources
  • Source appropriate patients in the community through meetings with referral sources and patients
  • Meet or exceed home health referral and admission goals
  • Spend 80–90% of time in the field
  • Manage and plan an independent schedule around relationship-building and results

Requirements

What you’ll need
  • Associate or bachelor’s degree in Marketing, Business, or health-related science preferred, but not required
  • Experience in healthcare sales/community marketing with a verifiable network of accounts
  • Home health and/or hospice agency experience preferred
  • Exceptional communication, negotiation, and relationship-building skills
  • Knowledge of Medicare guidelines for home health is a plus, but not required
  • Tenacity, empathy, emotional intelligence, and a solutions-oriented mindset
  • Licensed and insured driver with reliable transportation
  • Daily travel is required
  • Clinicians (therapists/nurses/social workers) are strongly encouraged to apply
