About the role
Key responsibilities & impact- Provides physical therapy services according to a written physician’s plan of care
- Assumes responsibility for interdisciplinary assessment and development of individualized plans of care
- Provides patient, caregiver, and family counseling and education
- Prepares clinical notes and progress summaries
- Communicates with physicians and other healthcare practitioners regarding the home health plan of care
- Assesses therapeutic, rehabilitative, and functional status, including home environment, muscle strength, mobility, gait, and range of motion
- Directs physical therapy treatment and determines priority needs
- Instructs patients, families, caregivers, and other personnel in physical therapy programs and therapeutic appliance use
- Reports patient reactions to treatment or changes in condition to physicians
- May train patients in the use of prosthetic devices
- Identifies and refers patients for other home health services
- Participates in case conferences, discharge planning, peer consultation, quality improvement, and in-service training
- Supervises Physical Therapy Assistants according to organization policy and state regulations
- Instructs, supervises, and evaluates home health aide care every two weeks when therapy is the only skilled service
Requirements
What you’ll need- Licensed to practice as physical therapist within the State, if applicable
- Passed an examination for physical therapists approved by the state
- Possesses a degree from a baccalaureate or master’s program in physical therapy approved by CAPTE or any successor organization
- Two years of appropriate experience as a physical therapist; community/home health experience is preferred
- Demonstrates good verbal and written communication and organization skills
- Possesses and maintains current CPR certification
- Ability to drive and/or have access to transportation for agency-related travel
- Must have and maintain a valid driver’s license
- Must maintain automobile insurance coverage and have access to an automobile
